The nose is one of the main features of the face. Therefore, its harmony with the rest of the facial elements is important from the aesthetic point of view. Some people are not comfortable with their nose, either because of its size and/or shape, or because it is not proportional or harmonic with the rest of the face. In addition, it is not uncommon for it to be associated with functional alterations or problems breathing normally through the nostrils. The origin of these alterations may be congenital, or due to nasal trauma or previous illnesses.

What is rhinoplasty?

Rhinoplasty is a surgery that allows the correction of aesthetic and functional alterations of the nose, either by changing its shape and size or by remodeling specific areas of it (dorsum, tip...). Sometimes certain external deformities and functional alterations are associated with deviations in the nasal septum (septum). This is treated by means of a surgery called rhinoseptoplasty.

The nose is not only an aesthetic element in the face, it also has an essential function: breathing. Due to this, it is essential not only to achieve a harmonious and proportional nose with the face, but also to be functionally correct.

What does rhinoplasty surgery consist of?

As far as surgical approaches are concerned, there are two types of rhinoplasty:

Closed: it is performed through incisions that are hidden inside the nose. It is indicated in those cases in which it is not necessary to make too important osseous or cartilaginous modifications.
Open: it is done through an incision at the level of the columella (area of the nose that separates both nostrils), allowing us to have a direct view of the cartilages and nasal bones. The final scar is imperceptible. It is indicated in those cases of greater complexity in which we must make relevant modifications to the nasal bones and cartilages.

We employ a procedure to address the nasal structures called preservation rhinoplasty. This technique has been mainly promoted by Dr Baris Çakir in Istanbul. Dr Ojeda has had the opportunity to attend international congresses and courses focused on preservation rhinoplasty and thus learn from the best.

This type of intervention consists fundamentally of:

Preserving and restoring the nasal ligaments: in this way we achieve maximum definition and naturalness in the tip.
Dissection in the subpericondrium and subperiosteal planes: the nasal bones and cartilages are wrapped in a membrane; in the preservation technique the dissection is performed below the membrane (subperiosteal and subpericondrium planes) where there are no blood vessels, generating less inflammation and bruising.

Main advantages of preservation rhinoplasty:

Maximum definition and naturalness in the tip.

Less swelling and edema.

Less injury to the nasal structures.

Once the approach is done, we access the cartilages and nasal bones, to remodel them and make the necessary modifications. It often requires a reduction of the nasal dorsum and approximation of the bones to obtain a less prominent nose in its profile and narrower in the frontal view. Similarly, it is often necessary to work on the nasal tip, reducing volume when necessary or simply improving the contour and definition of the tip.

Pre-operative study

At Clínica Ángela Ojeda we always guarantee the greatest security when carrying out all interventions. Therefore in the preoperative consultation we carry out:

Detailed clinical history and physical exploration, with personal antecedents and previous pathologies.

Nasal functional study.

Conventional analysis with coagulation study.


Chest X-ray, sometimes required.

In addition, in certain cases of deviation of the nasal septum, we will request a scan (CT) for a thorough understanding of your anatomy prior to surgery.

All these tests will be assessed by one of the specialists in Anaesthesiology and Resuscitation who are part of the team, who will indicate whether your state of health is optimal for carrying out the operation with total safety and guarantees.

How long does the surgery take?

In order to guarantee the safety and well-being of the patient, the interventions are always performed in the hospital, in optimal conditions of hygiene and sterility.

The surgical procedure has an estimated duration of 1-2 hours, depending on the technical needs of each rhinoplasty. The anesthesia used is general and the time of admission to the hospital where we perform the intervention is 24 hours.

How is the recovery after a rhinoplasty?

The first hours

At the end of the intervention we place some plugs in the nasal cavities, which are removed 24-48h later. You will also wear a splint externally for approximately 1 week, in order to help the skin adapt to the new framework and protect the nose from unwanted movements of the nasal bones.

The first days

The post-operative period of a rhinoplasty is not particularly painful, but rather annoying due to the difficulty in breathing associated with the plugs and internal edema. For this reason, we will arrange for the necessary analgesia, which will manage to control such discomfort.

All cures and check-ups will be carried out by professionals who are part of our medical team. To guarantee the quality of the result, we individualize each post-operative process with personalized indications. Therefore, we consider it essential for a correct recovery and excellent results to follow the guidelines set by both the doctor and her team.

The first 3 - 4 days

The first 3 - 4 days after the surgery it is normal to present swelling in the face, especially in the nasal and periorbital area. After 7 to 10 days, the appearance is usually good and you may be able to return to work. Non-impact sports may be progressively resumed after the 4th week, always with your surgeon's consent. You will have to be careful and avoid sports in which you can receive facial impacts until 2 months after the operation.

The desired aspect is not evident right after the surgery, you will have to wait about 3 weeks for the swelling to go down and for approximate results to be observed. However, the final results will be progressively verified, normally by the 3rd month there is little residual swelling, being definitive around the 6th month.
